You will be contacted for a brief assessment and qualified patients will receive a letter (or another applicable document) confirming an endorsement for an emotional support animal via digital delivery.
* Note: Price for assessment is for one animal and one letter/document only. Assessments for the need for additional animals must be purchased separately.
*Valid for residents of CA, AZ, CO, FL, MI, or WY only.
Requirements & Eligibility
Qualifications
Only patients diagnosed with a legitimate mental condition qualify for an endorsement for an emotional support animal. If you do not qualify, your fees will be refunded.
Confidentiality
Protected health information will only be released directly to the patient unless a signed authorization to release to another party is provided.
